I stopped playing this game months ago due to performance issues and I wanted to give this game another try today as I really hoping performance is improved/fixed in recent patches. Even on medium settings with AA off, I am getting 30-40 FPS with horrible screen stuttering, 100% GPU/VRAM load causing the temperature to rise 75° + celsius and GPU fans speed to noisy 3000 rpm.
Games like POE and Doom Eternal run from 100-144 FPS capped on a high setting without running too hot.
My hardware specs:
CPU: Ryzen 5 3600 with 16 GB Ram
GPU : AMD Radeon RX 480 8GB RAM
Unfortunately you are one of the unlucky ones that are most affected by the games optimisation issues. IMHO, its a long way from being good or fixed, and things improve with each patch but there are still quite a few people who are struggling even though their systems (on paper at least) are on par with others who are not having such a big problem. Other than the obvious GPU requirements, the game is also very CPU intensive at the moment so its a combination of factors that are causing poor performance and its very difficult to say what is definitely causing problems for some people more than others.
For example, I run i5-7500/1060/16gb/NVME @ 1080p all settings on lowest possible with vsync. I have 75fps in town, 60fps in most maps, dropping to 40-45fps in busy moments and 20fps if I dont watch out for known fps killer problems. The only change I made on my system is a more aggressive than normal fan curve to keep my GPU temps down (I prefer things much lower than most people). The game looks fine on lowest settings and I can play and enjoy it. My CPU & GPU hover in the 40-80% utilisation and temps stay below 75 degree c.
On paper (userbenchmarks), your CPU is between 10-30% faster than mine and your graphics card is about 10% slower… so…
Assuming your system is sorted wrt. driver updates, system is clean, good airflow and not running anything in background etc then my only suggestion, for now at least, is to reduce resolution, graphic quality settings and enable vsync (or frame limiting if you are using a higher Hz monitor). This should bring down your CPU/GPU temp and at least allow you to play the game at a semi decent frame rate of closer to 60 fps.

ps. Known moments for fps problems are:
Large numbers of mobs - i.e. dont do mob pulls,
Skills that proc all over the screen (poison pools, lightning novas, orbs etc) - try and spread things around rather than try to wipe lots of mobs at once in one spot. Not saying dont use the skills, just be aware that they slow things down if you are attacking lots of mobs - see above.
Builds with lots of minions (skeleton etc).
pss… For Devs: Anecdotal comment: it seems like people using older Ryzen/Radeon combinations are struggling more than others with regards low fps.
Thanks for posting a detailed analysis with tips and suggestions. I tried various methods, and unfortunately, I can only play at a decent 50-60 FPS is when I changed the graphics settings to “Very Low”.
With medium/high, I get a low 30 FPS with high GPU, VRAM usage as soon as the game is started and sitting idle at the character select screen. To me, it suggests the game engine itself is broken or has major perf issues if it can’t render a fairly static character select screen without using 100% of resources.
Yip… Very low is about where I have settled playing mine and quite honestly its perfectly playable 60-40fps… I am not one for much eye-candy anyway so I am happy for now considering LE is effectively still a beta product…
Medium & High are impossible (i.e. not playable) on my system (I’d expect them to be impossible on yours) and LE runs away with a GPU if you dont limit it in some way… Its like a runaway train and will just pick up speed till it jumps the rails.
Especially older hardware but it does it on the latest and greatest 3090’s too - they just go from 120+ fps to 60fps where the rest of us drop from 60 to low 20s or less. and yes, your observation that the game has performance problems is true… Its no secret…
At the moment the devs seem focused on content, balacing & multiplayer… For good or bad is debatable and depends on who you ask.
I am optimistic that they will eventually get to the performance problems - mainly because EHG havent let us down yet and they havent said that they are focussing on optimisation yet… just minor improvements as they go so far… Am hoping once they can focus on it, things will improve for everyone.
I hope that they will work more on performance
I’ve left Poe and switched to this game because the bad performance in Poe killed the fun for me.

I’m playing in Wqhd with 72Fps capped… Fps dropping to 40 when a lot of stuff is going on on the screen. Sometimes it feels very clunky.
Lowering shadows or anything else in the options doesn’t make so much difference
Your system spec is about 50% faster CPU than mine & 30% faster GPU depending on the task, so I figure at 1440p resolution you should probably be getting 35-40fps in heavy moments when I get about 20fps. I am guessing your normal play average is probably 50-70fps. Which based on what I have seen is probably “normal” right now.
Setting all graphics settings to very low/low will probably help you get smoother play and help keep things stable. Things like antialiasing make a difference but the others are less obvious unless you go from Ultra to Low… Shadows performance also had some work in a previous patch so its not that obvious a difference when changing it - for stability best to leave them off.
