I keep crashing. MY 2nd monitor usually goes out first followed by main one then grey screen and crash. It used to happen on bosses. Now that I have finally passed a few of them that it happened on it just randomly happens as well as on bosses. I am just trying to go through campaign on new character.

Sorry you are having hassles…
The game is in beta so things like this unfortunately can and do happen… thankfully there are a few things to try…
Without more info on your system spec & in-game settings here are some general things you can try… EDIT: ah I see you added the Dxdiag, now just need the in-game settings?
Try the following in the meanwhile:

Verify the game files in Steam - crashes do happen if the files are corrupted and it does happen.

The game has performance related instabitlity… You didnt provide your in-game settings, but are you trying to run things on High or greater settings at the 2160p resolution? Your graphics card 980 is about 15% faster than my 1060 and I have to play the game on very low settings at 1080p just to have a decent 60-75fps and a stable experience. I am going to assume that you are not trying the game on its lowest settings so try the following:
Temporarily set the game on 1080p, 60fps framerate limited (NOT vsync) and ALL in game settings to very low or disabled… See if the game is more stable in this mode… If it is, then you are probably suffering from the performance related instability and have to be more conservative on your settings. Increase the resolution to what you want to play at… 2160p is a very high res for your graphics card and I dont recommend it - typically people with 3000 series / amd equivalents play at that res… Test again and monitor your GPU usage… if things are stable, then you can consider increasing the FPS limit but honestly I would leave it on 60fps especially if you want the high resolution… Next, is to increase the in-game graphics settings one by one and testing as you go… Normally I would suggest not going above Medium but you may have to do what I do and keep things on a lower setting to get better FPS and stability…
Only slightly related and out of interest I did a quick bottleneck check on your CPU/GPU combinationi - its not gospel but your CPU is far too fast for your GPU so its likely that you are going to get strange limits or odd performance results depending on if a game is more CPU or GPU bound… I understand with the GPU shortage there is little to be done about it but just keep that in mind when playing games - your poor GPU is probably working its arse off and might not be too stable for it so be gentle with settings…

Another thing I forgot to check…
At the end of your dxdiag there is a section on errors your system is getting… One in particular is not healthy - LiveKernelEvent 117… This is tricky to be sure of the cause but it can be something to do with your GPU & its drivers… I would suggest you check your windows Error logs and see if there is anything that you need to check / fix there… This could very well be causing LE to crash too… You may have to do a clean driver install (window safe mode etc) and you might need to make sure that your windows system files are ok by running system file checker
