In the Empowered Fall of the Outcasts timeline, The Altar of Flesh is shown with a (?) icon. My stability bar is filled beyond the boss node, with the boss node on the bar showing yellow. When I attempt to enter the boss echo, the button to enter does nothing.

Completing the two preliminary quest echoes in order unlocked the third (boss) echo. In other words, it’s as if the quest echoes in the non-empowered timeline were never completed.
