When you alt tab it also switches the map name in the top right corner. Both screenshots made at the same time but with alt tabbing once between each picture. Only solution is to exit to character selection screen.
I have also encountered this bug, after dying on The Frozen Roots you seem to spawn outside of the map unable to move in the same location. However, I have not encountered the map name switching when using alt tab.

I’ve encountered the same bug, unable to move/ cast spells/ teleport out after re-spawning off the map.
+1 to this. Happened to me and my party of 2 more. Can’t use map or anything. I only escaped using Lich + Reap
