This happened yesterday, so do not have a screenshot.
I was dragging an item to my character and accidently clicked the spot to zone into a new area with the item still on my pointer. After i loaded into the new zone, that item basically was a ghost item on the screen for the remainder of my session. It would attach itself to other items as I drug them around as well.
