Hey there… this is a known problem… Devs havent confirmed exactly what the issue is, but it seems like its loot filter related but its not repeatable on demand making it hard to trace.
If its still valid (not overwritten), please can you post your player.log file from the session when this happened - there is a related text debug message that seems to be in most peoples player.log when this happened.
