Whenever an Orange item (Unique) has dropped it falls under the floor and I cant reach it, its happened twice now. Once from a mob I killed and another from the Erzas Ledger / Artems offer questline.
That’s about when I stopped playing since I didn’t feel like losing out on these items continually. Hopefully this is a simple fix but eh, its also not the item label toggle btw, read a different post about that. The item clearly falls below the floor and the pick up prompt does not appear.
