Umbral Blades "Precision Cuts" not stopping on target hit

I am using Umbral Blades with Precision Cuts, Sword Thrower, Cacophony of Steel, and Steel Torrent.

Precision Cuts sometimes does not activate and passes over targets even though it lands a hit (could be cursor location, I’m not 100% sure what target location is.)

The video provided shows this on spiders, but it happens on all enemy types and sizes.
