I died fighting Lagon. So after reviving, I spawned at the portal to enter the boss fight. I created a town portal to go back to town, but I couldn’t use my town portal to return to where I was. The portal spawned inside a building in town.

Here is another location where the town portal spawned somewhere inaccessible.
i too have had the same issue, multiple times. same location as MyLith
I have the identical problem with the Town Portal in Thetima Divine Era.
