Tombs of the Erased BUGGED - cannot progress

Twice today I have entered Tombs of the Erased with the following 2 bugs;

  1. Mini-map disappeared.
  2. Defeating the boss at the end of the Tomb did not open the door/barrier at the end of the level. I got stuck in the tomb and was unable to progress. I exited the game, and rebooted it… Tried another Tomb, and the same issue persisted. Boss door did not open.
