In many of the quest echoes in the Monolith, you can spawn in with mobs surrounding you. If you have an evasive ability you can often extricate yourself without issue, but if you do not or if there is lag as the level loads, you can die very quickly, sometimes before you actually can move your character. In effect, you spawn in dead.
I’ve only noticed this in quest echoes, not the generic echoes.
I can provide an example - the caravan one in the Ruin of the Empire monolith does this.
