Stuck points of Titan's canyon and Necropolis of the deep

Titan's canyon

Stuck in the wall.
But can escape from here using portal in map menu.

Necropolis of the deep

Once stand on this point, can not move in farthest camera.
Can move character by Zooming camera, but this is unnatural.
