Full offline, happened both normal and empowered timeline.
Killed all tentacles and then there is no new tentacle spawn, and Lagon won’t drag me back to the phase 1’s stage.
Fixed after restart game but will happen again next time.

Full Offline, Empowered Mono, Stuck in 2nd phase: no more tenatacles spawning, the moonshine in the middle does not move.
Update: after restart, I kill him the first time without any issue. If I then go at him again and die, then he is bugged → repeatable. 1. Kill Lagon, 2. Die at next Lagon, 3. Bugged in 2nd Phase.

Killed all the tentacles in phase two - not teleporting back to Lagon.
Edit: New bug - Reset the game. Killed Lagon in the second phase, then killed the Harbinger. No completion notification, everything dead, just stuck.
