Stuck in donjon, no exit error LE-69

hello,
we are Stuck in donjon, we got LE-69 Error.

there are only 2 exit and we got on both this message.

the dungeon is soul fire bastion. We fail the first time, when we enter the second we got this bug. Maybe the first isn’t close when we both die…
