Flames of Midnight’s Spirit Step will get you stuck in Cemetery of the Erased (Tombs probably too) if you jump from the left side of the fence into this spot (see screenshot), once inside you can not jump back out.
Teleport/leave game are the only options.
This is 100% reproducible.
PS: I tried the in-game bug reporting tool, but it seems it doesn’t work fully offline.
