I use smoke bomb for my rogue and I have been ported outside of the map on several occasions in monoliths… tried to upload a picture, but I apparently don’t have the right file type. I have also been smoke bombed “into” a hill, under the graphics of said hill. 
No way to portal out of any of these situations, have to restart the game.

Are you sure it was smoke bomb not shift? Smoke bomb doesn’t move the Rogue.

Yes. Under Skills and Specializations, Smoke Bomb skill point “Escape Tactics” states “You backflip away from target direction when you use Smoke Bomb”…
The multiple times that I have been smoke bombed outside of the map parameters, I have backflipped into the abyss.
