Shattering rune : Counter not updating AND shattering created a unique

Hello,

I used a lot of shattering runes in a short amount of time.
I noticed the shattering runes counter only updated when I logged back to character selection and entered the game again.

Also, at one occasion, a shattering rune turned a 1h scepter into a 2h unique spear.
It even made some kind a “crack” noise I’m not familiar with.
I’m 100% sure I did not use a rune of ascendance.
