Scorpion Shrine Didn't Spawn Anything

What went wrong?

I entered an echo and there was a shrine near the spawn point with ‘summons golden scorpions that drop rare items’. Clicked it and nothing happened. See screenshot w/loot filter disabled+empty inventory as me trying to provide some kind of evidence.

Blood, Frost, And Death timeline, not empowered.
