Not 100% positive what is causing it. but Apprentice was on stream and he used transplant into a pack of enemies and they killed him but the health that he gets back revived him. the respawn screen showed up and he could continue to fight and open passives as well as inventory and interact. after he cleared the wave he clicked respawn and was back alive in the wave. Then clicked continue and could keep going. we think it was transplant that caused this but not 100% sure. drink deep in combination with the rip blood nodes and transplant itself.

If anyone else has this happen, a log file would be very helpful. You can attach it to a forum post using the 
button. A link to a screenshot of the skill tree might be useful as well.
I will see if I can get him to try and get the log file if it happens again
