Do they apply on every tick, or only the initial cast?
For example: Will the Sorcerer passive Arcane Insight have a chance to proc every time Disintegrate ticks, or is it only when I first hit the button?
Neither because Disintegrate has the DoT tag & thus can’t hit.
That said, if the passive/proc says “on hit” then it can be applied on every hit of a channelled skill. If it says “on use” (or “when you use”, etc) then it only occurs when you start channelling.

It is important to disdinguish “channeled” and “on-hit”.
They are two different things, but they can be on the same skill.
For example Flurry can be turned into a channeled ability doing countless of hits during its channel
Disintegrate like in this example does not hit at all. (But Disintegrate has nodes in its skill spec tree that can cause hits while channeling though).
