Question about the "Nearby" mechanic in regards to damage sources

So I couldn’t really find an answer to this but how does the “Nearby” mechanic (for example the Ursine Strength node or Bastion of Honor) work in regards to damage sources?

Some enemies create damage sources which are independent from their body and do not require them to channel. For example the Ice Goliath ice spike things on the ground that explode, the Diamond Matron beam (if it is a hit and not a DoT - I am unsure), some of Orobyss’ attacks or Julras attacks and so on. They are not targettable or totems or something like that but deal damage (in this case I am talking about “hit” damage, not DoT damage!) without the source having to channel or anything like that, meaning Orobyss, Julra or whoever did the attack can move and use other abilities freely while the previous one still exists. In such a case, is the entity that did the ability still the source or is it the ability itself?

I am guessing the entity that used the ability is always the damage source.
This would make sence as ranged enemies can move or fire another projectile while the first one is still active.
But in the case of those “lingering” abilities I would like someone who knows for sure to confirm this.

Bastion of Honor has a graphical circle on the ground when you equip it showing you the radius it works in. But it’s about the size of the circle around the training dummy at the end of time.

Yes I am aware of that but that wasn’t my question.
I am assuming that the damage source of a “lingering” ability that deals hit damage is always the entity that used the ability (as long as that ability does not create something targettable like a minion or totem or something of course) and I want someone to confirm this.

Yes, and if a skill/whatever has a duration it’s stats are set when it’s cast.
