Prophecy Reward Didn't Drop

I have a Prophecy:
Event: Death of Majasa
Condition: The Chamber of Vessels
Reward: 4 x Unique Ranged Weapon

I killed Majasa, twice, and nothing has happened? Am I missing something? Do I have to hand it in somewhere?

I’ve also had a prophecy not drop its item, totally different requirements/rewards though

This is just a guess, but I think both times this has happened to me now my minions had killed the thing to trigger the prophecy offscreen

Having this issue multiple times. Seems to be weavers will prophecies have it happen the most.
