I was running monolit for the fall of the outcasts last night and the boss at the end of the echoes fell under the floor and I could not collect the loot.
Here is the image of the fight. all his attacks but the aura could still hit above ground.
Edit: He dropped as soon as the fight engaged before i got off an ability.
