This is great analytical work. How did you log the portal crash in offline mode? I hope LE developers are paying attention to this post.
There’s a “Known Season 4 Issues” channel in the Discord, with a message that mentions " * Sometimes offline portals fail to work in Echoes", so they are at least aware of the issue. (I wouldn’t have used the word “sometimes”, though… it’s more like “most of the time”).

This is just the default log Unity writes, the two most recent ones are located in a directory for the game on your computer (the path for Windows should be [User]/AppData/LocalLow/Eleventh Hour Games/Last Epoch/) as Player.log and Player-prev.log.

Yesterday i tried to play but with leaving each time as a solution (either leave or restart the game). I couldn’t find any pattern for the recurrence.
After a fresh start of the game I’ve got to do 5-7-5-2 echoes before the bug (the 2 is really surprising and frustrating). And after leave I’ve got 3-6-4-5-11-7-4 echoes.
Perhaps there is a link with the type of reward, loading or an other thing that cause the boolean value of the portal to not activate but it’s really jarring to tried to play with a Damocles sword above your head.
I Hope a fix will come quickly cause RN i can’t play empowered mono if I’m not sure to get some reward for specific echoes (and echoes chain).

On top of that, as soon as I die in the Molmnolith, I stay on the map.

I have found that if you wait atleast 25sec after you spawn tp it works for me. I am at 25 in a row. hope it helps.

That’s an interesting observation, but I’m doubtful it a tp on timer issue. Also, I don’t think we should have to wait 25 secs to get out, though. That put’s you in a real bind if you have to portal out to get out of a bad situation. 25 seconds is also a long time to wait relative to this type of gameplay.
The latest game patch 1.4.2 doesn’t fix this portal error either

I was hoping the latest patch would finally fix the bug, but it didn’t, and now I’m in total despair.
This is my first season playing, and I was genuinely starting to love the game. But because of this bug, I’ve completely lost the motivation to play anymore.I would like a refund.

I love this game and I’ve been a loyal supporter since the early days, but my patience has finally run out. Why is such a critical, game-breaking bug still not fixed? What are you even focusing on? You’ve forgotten how massive the end-game community is. How are we supposed to farm when the basic mechanics are broken? This is incredibly poor management and frankly, unbelievable.

Same issue here. Really disappointed that after a 6 month break and 300+h in the game, I’m getting pushed away again. LOVE THE GAME, but not sure if I’ll puck it up again.

Also happening to me in offline-mode. This is it for me for this season and to be honest likely till the game is at least playable and 90% bug free.
The issue is still happening…

Update: as of the latest patch I’ve done like 5ish Echos and it hasn’t happened yet. Usually it happened after 1/2 echos, after restarting the game/relogging into character, so, possibly fixed?
EDIT: Yep, bug is still there, just takes a few more echos to appear, I think it was something around 8 echos when the Portal stopped working.
