I just realized after placing the Threshold of Eternity that my quest for picking up the Fractured Prison Woven Echo never counted as being completed. I beat the Omen’s Veil Woven Echo and picked up the Fractured Prison and then proceeded to beat it as well and none of it counted towards my quest. I also just tested running the Omen’s Veil again and picked up a new Fractured Prison and it still does not complete the quest.
