From time to time (maybe 1/h) Permanent Bond will summon another wolf companion even though one is already summoned. In some cases this will cause an existing companion to be unsummoned (if at the limit), in others it can cause the companion limit to be exceeded.
Update: I noticed this happens mostly during area transitions.
