Overlapping prophecies failed to produce even one valid reward

I currently have 3 “Death of Emperor of Corpses” prophecies, each should give a 16 Weaver’s Will armor piece.

I just killed the Emperor with the appropriate corruption to fulfill all prophecies. Each of the 3 gained a fulfilled (each has 8 uses). Only one WW item dropped - a 3WW belt.

Just happened again. This time I received 2x 16WW items, but it should have been 3.
