Offline save files disappear

I have experienced two different types of loss of save files.

  1. The save file still exists but the character is not showing up in game.
  2. The save file is completely missing.

I am aware there was some issue where save files were being overwritten when creating a new character but in my case file are disappearing even when no new characters are being created.
