I had just completed the quest and the dialog option to open portal appeared, but as i was killing some other mobs I then got randomly killed and was ported back to the staging area and losing progression.
I then went to go into another echo, to only find this was total void of all foe and attacking and only way to exit was to port back to staging area which resulted in further echo rollbacks.
Yeah, that’s a known issue if you complete an echo but also die.
Another way this has happened to me was dying from the boss mechanics, minions kill the boss, “Open Portal” button appears, and is then clickable.

Yeh I also had it where even after the portal opening another wave of attacks happened
