[Monolith] Flame spire spawns outside walking zone

The minimap does not match the real map and the flame spire is not in an area where I can go so I had to leave the echo

I have the same issue cant progress in monolith. I exist out and restart but keep getting this same map.

Monolith of Faith - Volcanic City - Cleanse Forge - Enemies spawn behind obstacle

On the map Volcanic City of the Monolith of Faith level 62 timeline I had the “Cleanse the Acid Forge” quest. Enemies would spawn behind a fence, where they could not move. At first I didn’t even see them, thinking the forge was somehow broken, because there would not spawn a new wave. https://imgur.com/fkiWJ4C Here you can see a hit point bar in the lower left corner of the image - this enemy cannot move out of there (but in this case can attack me with a ranged poison attack).

Volcanic City Map - Forges spawn in walls & Spires are off map

Hey… On a least 3 separate times I have had a problem where the updated Volcanic City Map in MoF has positioning problems. Had a 3 spire goal only to have the last spire being out of reach in the top right of the map where all the open lava areas are. Had a forge spawn halfway in a wall so the mobs that spawned from it, were effectively off the map on the one side and unless I spammed an AOE , I could not hit reach them or part of the loot that dropped.
