I used this option for some types of idols (class specific)
And it was the only condition.
After that random items dropped by mobs are capitalized.
When I pick them up and then drop again, they are back to normal font size.
I only have daggers emphasized, but random loot can also get emphasized while some daggers don’t.
So yeah, something is bugged there.
Yeah, emphasize is still bugged. Found a post from 1 month ago that the Devs know about it, but I guess they haven’t fixed it yet
