I just logged into a char I didnt play for a long time and my Helmet looks like this:

It’s a legacy item, yes. You can tell by the fact that there’s 4 affixes but two of them aren’t noted as either prefixes or suffixes.
Correct this is a legacy item. The art was old and doesn’t exist anymore.
