I will be looking at items, and the game will randomly switch out items when looted/picked up. Another issue is when I try and put an item into storage, it sometimes puts my equipped item in instead and equips the item I was trying to store. This has caused me to sell the wrong item, or dismantle the wrong item due to the switch happening without me realizing it until after the fact.
