I just ran into an issue while playing in offline mode that items were dropping, showing their pillars of light of various colors but no labels were appearing. Even while holding X to disable the active loot filter none of the items were appearing and were not lootable off of the ground. I noticed when a unique dropped. So far in one session I missed out on a unique, a set item and a few purples., 
I should have thought to take screenshots.
The same issue in Online Mode, even prohibiting filters is ineffective. It can be known that this bug is sporadic. Once it happens, you must either go back to character selection or re-enter the game to resolve it.My friend refunded because of this.
It sounds like you accidentally turned off item plates. Try pressing alt+z
Thanks folks. It hasn’t happened since. I may well have hit Alt-z to hide the tiles by accident for all I know. I didn’t try re-starting from the login screen. I exited and re-started the client and all has been fine ever since.
