When hovering over item on the ground, the description window disappears, if new items drop.
This can happen at the end of the echo, when you pop echo rewards and then the reward chest, then hover to look over echo rewards, but unless the reward chess is done dropping items (and it can continue dropping over 3 seconds), it’s almost impossible to look at item descriptions, as they disappear almost immediately.
It also happens, when an items drops, and then minions or other things kill a monster that drops loot.
Or with the shrine, that drops idols, often a good idol is filtered at the beginning, but I cannot look at it’s description until the last idol drops.
