I think this is a bug with the Swipe skill rather than the items in question.
I have tried Peak of the Mountain, Boulderfists, and Xithara’s Conundrum. None of these items alone, or even when equipping all three, stop Swipe from leeching health.
This has occurred consistently while wearing the Exsanguinous and Last steps of the Living setup.
