Teleporting using specialized summon skeleton mage while exiting a monolith through a portal. I ended getting telported somewhere completely off the grid. All I could see was a massive blackhole and red background with nothing else within even expanded map.
Easily fixed by just teleporting to a waypoint but I thought it was pretty funny.
