Hungering Souls occasionally casts less projectiles than it should

Occasionally (about 1 cast in 10) when I directly cast this skill, it casts less Souls than it should. It can cast any number from 1 to 5 basically, seemingly at random, though when it goes wrong it is almost always only a single Soul that is cast.

The only other skill node I have that is interacting is Devouring Release in the Death Seal tree, however I see this bug even when Death Seal is not active. I have no points in the Vengeful Souls node in Hungering Souls tree.

want to second this. a lot of the time my hungering souls casts less than 5 souls and most notably only 1 fairly often. Though the only other skill I have interacting with mine is 5% chance to cast on reap kill in reaper form.
the only nodes that I have directly influencing the count are forbidden pact, soul swarm 1, and all of risen cemetery.

In some specific places like boss fight right after Cultist Camp in Ruined Era. There is some purple goo geometry which appears for a moment when the boss spawns. It seems to correspond to area where spell projectiles aren’t spawning - on both images, character is casting projectile spell: before/after the boss spawns. It is not limited to hungering souls and this particular location. I experienced similar issue with skeleton archer projectiles and in different locations.
