How to get a fifth affix per item?

Hello guys,

I remember that it is possible to add a fifth slot on exalted items but I can’t remember how to do that, can someone help me recover my memory please ?

This has nothing to do with exalted items.

The extra affix is “sealed”, which can be a prefix or suffix. An Item can only have one sealed affix.
This is achieved by sealing it via Glyph of Despair

The lower the affix tier, the higher the chances for sealing and you can only Seal T1-4 affixes. (T5 Sealed Affix is exclusive to Rare Items only in Soulfire Bastion)
