Because that’s the monolith that gives unique helms as a mono reward. It’s a random drop so you can get it anywhere that’s above lvl 70.

Would it drop more frequently in empowered monos and/or higher corruption or does that not matter?
Probably yes because you’d have a higher chance to drop uniques. Higher item quality (or more items) → more uniques.

@Dammitt Just got one of these helms to drop, and with 1 LP too! But it says its Level 70 not Level 1 (per the Planner).
Uh, where does it say Level 1 in planner? It’s level 70, both in db and in planner.
I see. There’s a bug in combined item level somewhere, sorry for the confusion, this will be fixed soon.
And yet it shows the correct level for me, very odd.
It is fine in the Item Database, but not when equipped to a character.
Per Dammitt’s comment, I assume his code to combine an Exalted + Unique to make a Legendary appear in the planner is where the defect lies.
It was correct in the list of items both in planner and db, but incorrect when combined item is shown. In current implementation any item in planner is a combined item, even the one without any affixes. Item’s base level was incorrectly determined for unique items, which caused the issue. Now it’s fixed.
