Screen goes completely black.
Computer remains powered. Signal from video card to monitor is absent.
Monitor goes into sleep mode immediately.
Has only occurred when teleporting in Last Epoch.
Happens about once ever 1-2 hours. Hard Reboot required.

Update.
After writing this post, I went to play again (Steam) and the prompt to install game came up. I checked my library and it is no longer listed as an installed, playable game.
Could you upload your log file & crash file assuming there is one (which there may not be if you had to reboot).
And probably the Graphics.ini file if it’s still there after a crash.
I just had the same problem, while in an echo. Lost my both of my monitors signal and had to do a hard reset just to get my monitors back
