I completed the first level 90 monolith and fought the 1st harbinger. I then spoke to the Forgotten Knight in the Shattered Road. This started the Harbingers of Ruin: Part 2 questline. Since then I have completed every other monolith on empowered, now reaching up to over 177 corruption on a different timeline and NO harbingers have spawned. Something is broken and I am unsure how to proceed.

I had the same issue, mine was caused by me right clicking on the boss fight instead of left clicking. Were you using right click too?
Holy hell, you gotta be kidding me. That was it. That definitely should not function like that, thought i was losing my mind. Feel like i wasted a whole day. Thank you dude.
Holy god… I have now completed 20 monoliths trying to work out what went wrong. How this topic is not at the top of priority lists is astonishing. Multiple hours of trying every single monolith from 0 stability to 900, going back to level 90 monoliths and all sorts of stuff. Absolute outrage. Are we so few players that actually know about right clicking echoes that this just went unnoticed?

This is quite nasty hidden bug. Happens to everyone I think. Right click = no harbinger.
