During the run an off hand catalyst dropped that made a nice increase to my survivability I decided to improve it in the forge. At my some point my helmet was unequip with a different one. I had noticed a nice decrease in my damage but I figured it was because of the catalyst. During the Arena I got a Lightless Arbor Key and during that run is when I noticed my helmet was replaced with another. Log File Screenshots (via a site like imgur) or video, if applicable
Hard to say how this could possibly have happened…
I will admit that I have miss clicked and replaced gear and even dropped/sold gear I didnt intend to at one time or another… Using the forge I have mistakenly swapped out equipped gear for gear I was forging on and then only realised the mistake after the game auto equipped a new piece of gear i picked up… all without me knowing…
Perhaps something like this happened to you?
I cannot see anything specific in your log that could indicate an error that happened that could have caused this, but I would recommend that you verify the Game File Integrity through steam, just make sure that your install is ok…
Are you playing with a controller?
I play with a controller and sometimes, items I look at are automatically equipped when I close the inventory or forge. This did not happen before, when I was playing with keyboard and mouse. It really started when I started using a controller.
