I can’t login today so I have no screenshots of the build but you can find recreation of my build here:
I don’t have any talent that has Cast Nova on Target location. This bug happens occasionally and usualy after the teleport. After that I hold Elemental Nova key but Nova is detonating on my place of departure. My teleport has only Elemental Nova on Departure and Arrival.

I have noticed it also affect Flesh monsters with a big stone and a green telegraphed location also being displaced (facing my departure location).
