I killed Elder Pannion right as he was teleporting from the edge back to the center and he dropped twice his normal loot between the two places. He also played his death soundclip twice.
Yip… this is a known one… if you tag him just at the right moment it happens… Apparently its him dropping his specific loot table and then a second normal boss drop… Figure the devs will get to this eventually…
dont think anyone has mentioned his death soundclip repeating tho… 
It’s likely the same issue as Rahyeh doubling up on loot.
I’d imagine Liath can as well if she died at the right/wrong time.
