The druid skill Invigoration in the Werebear tree states rage gained on hit and kill. I am only regaining rage on kills and not on health. Not sure if I am misunderstanding or not, but wanted to post it. Deleted the character and created a new one and still happening.
EDIT: I have tested this with swipe and maul.
