Died in hc but got rolled back instead of becoming a ghost

So I just died in a lvl 90 mono, but my character and inventory got rolled back instead of becoming a ghost. I lost loot from maybe the last 30 minutes of playtime? I still have some loot I found in the beginning of the session but not all of it.

My progress in the monos got rolled back as well. I had cleared spirits of fire and recieved a blessing but that’s gone now. I got a 1LP Herkir’s Vessel from that fight and that’s gone as well.
