Dev decisions around unique items

Let me preface this by saying I have no experience in game design or development.

A thought that keeps popping up for me when playing this game, is how do the Devs decide what stats to put on a unique and why… It’s obvious some uniques are designed around a skill to be played with a certain build in mind (Herald of the Scurry), and other times it has some strange mods like Cleaver Solution that can used in a variety of ways. How intentional are these decisions? It would take too long to theory craft all possible options with certain uniques, so do they just simply put some stuff on and say “let’s see what the players do with this”, or is there a clear intention? Or a bit of both? I’m thinking both…

Why put attunement on Reign of Winter when the icicles scale off dex anyway?
Why put str, dex and int on Blood of the Exile?
Why put all res on Dragorath’s Claw?

Just a few examples… this list goes on.
