Happened a few times to me now (as a Lich). I see a loading screen when entering a monolith (I never click or press any buttons during this loading time) and the next thing I know I am dead because an AoE skill which has targeted one of my minions has spilled over onto me and killed me even though I should be invulnerable. I saw it once happen just as I entered the monolith.
