Crafting: How to change prefix and affix?

Hello,
got a “Reptilian Tusked Greathelm of Deflection” with the prefix “Minion Health Regen” and the suffixes “Dodge Rating” and “Physical Resistance”.
Is it possible to craft this one to a “Primalist’s Tusked Greathelm of Endurance” with prefix “Minion Critical Strike Multiplier” and “+ To Summon Wolf 60% Increased Minion Damage” and the suffixes “Increased Health” and “Endurance” ?

You just need luck.
Either remove an affix with Rune of Removal and replace it with the one you wish, or try upgrading it to a different one using Glyph of Chaos. (or the other way around regarding the names glyph/rune)

There are two ways to change a Prefix or suffix.
1: If it is lower than tier 5 you can use a Glyph of Chaos to randomly change it to an Affix that can go in that slot.
2: You can use a Rune of Removal to randomly remove a Prefix or Suffix, freeing up that slot to be crafted on.
